Most Everyone Qualifies for a Booster Shot
We will be providing MODERNA Booster Shots,
to qualify, it must have been at least five months since
your second dose of Moderna or Pfizer vaccine
or two months since your J & J shot.

NOTE: You can also register through the links below
for primary series (1st and 2nd dose) Moderna shots.

Due to COVID-19 protocols, please arrive at the time of your appointment to minimize the number
of people waiting and wear a face-covering

*Please wear a short sleeve shirt and bring your VACCINE CARD*

Reminder: 15-minute post-vaccination monitoring is required.
