The E-911 Disability Indicator Form is for disabled Marlborough residents. By completing this form, it will add your information into our E-911 database so that we can better serve you in the event of an emergency and you dial 911 from your telephone. This information is confidential and will only be shown to the dispatcher when answering an emergency phone call from your address.
To access this form, please visit the Emergency Management department web site and click on E-911 Disability Indicator Form. The second page of the form needs to be filled out and returned to the Marlborough Police Department by mail or personally delivering the form for processing.
